Enduring Power of Attorney forms

Nidus does not provide forms for an Enduring Power of Attorney.

Most people go to a lawyer or notary public to make an Enduring Power of Attorney.

Representation Agreement forms

If you want to make a Representation Agreement with ALL powers, you must consult a lawyer who will draft the form for you. Only a lawyer may sign a Certificate of Consultation for a Representation Agreement.  ALL powers means standard and additional/broader powers.

If you are interested in a Representation Agreement with only Standard Powers, you may contact Nidus about our Self-Help Kit for purchase.

There are no off-the shelf forms for Representation Agreements or the Self-Help Kit.

The Self-Help Kit does not apply to every situation.
