Representation Agreement forms
There are no ready-made or off-the-shelf forms for Representation Agreements.

Form for Representation with Additional (broader) Powers
You must consult a lawyer if you want to include additional powers in your Representation Agreement. The lawyer will draft the form for you. Only a lawyer may sign a Certificate of Consultation for a Representation Agreement.

Personal appointment with Nidus Staff – an appointment by phone or in-person to answer any specific questions and to finalize the set-up for the legal form.

The cost of making a Representation Agreement through the Self-Help Course is $75. Nidus also offers a spousal rate. Nidus does not accept payment until enrollment is confirmed. Fees are subject to change.
Enduring Power of Attorney forms
